Drugs recalled after clinic mix-up

Anti-hypertension drugs have been found packs of diabetic medicine at the Lady Trench General Out-patient Clinic.

 

The Hospital Authority said today Diltiazem was found in a pack of Glibenclamide this morning during checks by a pharmacist.

 

As the incident was related to human error the clinic management will impose disciplinary action.

 

Diltiazem has so far been found in nine packs of Glibenclamide. The clinic has contacted 63 patients asking them to stop taking the tablets and arranged for drug replacement.

 

The pharmacy has reminded staff to be vigilant and strictly observe safety procedures in the preparation of medication.
